2. Complete and Upload Application and Supporting Documents

Complete and upload Atria Cares application and any supporting documentation, such as past due bill statements, medical bills, lease agreements, etc. (accepted formats: JPG, JPEG, PNG, PDF).

Once the Atria Cares application has been completed and uploaded, fill out the form below and click Submit. Please note, we will not be able to review your submission without the completed application and supporting documentation from Step 1.
